OVERVIEW
Gynecomastia — or male breast enlargement — is a common condition that affects men of all ages. It occurs due to an excess of glandular tissue, fatty tissue, or both in the chest, resulting in a fuller, more feminine appearance that can cause significant self-consciousness and discomfort.
While the exact cause is not always clear, hormonal imbalances, weight gain, genetics, and certain medications are all known contributing factors. For many teenage boys, temporary breast enlargement occurs naturally due to hormonal changes during puberty — but when the condition persists into adulthood, surgery is often the most effective long-term solution.
Gynecomastia surgery is one of the top 5 most popular cosmetic procedures for men in the UK, according to BAAPS (British Association of Aesthetic Plastic Surgeons) statistics — reflecting how widespread and treatable this condition is.
Persistent neck, back, and shoulder pain caused by the weight of heavy breasts — often worsening over time.
Difficulty maintaining good posture, increasing the risk of long-term musculoskeletal issues.
Discomfort during exercise or physical activity, making it difficult to maintain an active lifestyle.
Difficulty finding well-fitting clothing, bras, and underwear — affecting body image and confidence daily.
What Causes It
Gynecomastia can develop at any age and is rarely the result of a single cause. These are the most commonly identified contributing factors:
An imbalance between oestrogen and testosterone levels — the most common underlying cause, particularly during puberty or later in life.
Excess body fat can increase oestrogen levels and lead to fatty tissue accumulation in the chest — sometimes persisting even after weight loss.
A family history of gynecomastia can increase susceptibility — particularly in cases where glandular tissue is the primary cause.
Certain medications — including some steroids, anti-androgens, and heart or blood pressure drugs — can trigger or worsen gynecomastia as a side effect.
Significant and lasting reduction in neck, back, and shoulder pain caused by the excess weight of large breasts — often providing immediate relief following recovery.
Smaller, more proportionate breasts that feel balanced with the rest of your body — making clothing fit better and physical activity more comfortable.
A more balanced, natural breast shape that many women report significantly improves their body image, confidence, and overall quality of life.
Improved posture and a reduced risk of chronic musculoskeletal conditions — contributing to better long-term physical health and wellbeing.
Why Consider Surgery
For men who have struggled with gynecomastia — particularly those for whom diet, exercise, or medication changes have had little effect — surgery offers a permanent and highly effective solution.
A firmer, more masculine chest profile — with results that are immediately visible once initial swelling has subsided.
Many men report a significant improvement in self-esteem — feeling comfortable in fitted clothing and going shirtless for the first time in years.
When combined with a stable, healthy lifestyle, the results of gynecomastia surgery are long-lasting and generally permanent.
Advanced surgical techniques allow incisions to be placed discreetly, ensuring scars are well-concealed and fade significantly over time.
Surgical Approach
The most appropriate surgical technique depends on the nature of the excess tissue — whether it is primarily fatty, glandular, or a combination of both. Mr Juma will assess this during your consultation and recommend the most effective approach for your anatomy.
For Fatty Tissue
Where the excess is primarily fatty tissue, liposuction alone can achieve excellent results — removing fat through very small incisions with minimal scarring.
For Glandular Tissue
Where firm glandular tissue is present beneath the nipple, direct excision is required to remove it effectively — liposuction alone cannot address this type.
In many cases, a combination of both techniques is used to achieve the flattest and most natural result. The right approach is determined individually — based on your specific anatomy, tissue composition, and goals — during your consultation with Mr Juma.
The Surgical Process
Gynecomastia surgery is a well-established procedure, typically completed in a single session. Here is what your journey looks like from first consultation through to your final result.
Mr Juma will examine the chest, assess whether the tissue is primarily fatty, glandular, or both, and determine the most appropriate technique. He will be open about what is realistically achievable and walk you through the procedure, risks, and recovery in full — before any decisions are made.
The procedure is performed under general anaesthesia and typically takes around 90 minutes. Depending on the assessment, Mr Juma will perform liposuction, direct glandular tissue excision, or a combination of both. Incisions are placed as discreetly as possible — typically along the border of the areola — to minimise visible scarring.
Gynecomastia surgery is typically performed as a day case, meaning most patients are able to go home the same day once they have been assessed as comfortable and stable. You will be given full aftercare instructions and prescribed pain relief before discharge.
A compression garment must be worn for 6 weeks following surgery to reduce swelling, support healing, and help the chest settle into its new contour. This is an important step in achieving the best possible result and should be followed closely.
Most men return to desk-based work within 1–2 weeks. Exercise should be avoided for 2–3 weeks, with a gradual return to full physical activity after 4 weeks. Mr Juma will monitor your progress through follow-up appointments and advise you on when it is safe to resume each activity.
Recovery Guide
Gynecomastia surgery has a relatively straightforward recovery compared to many other procedures. Here is a general guide to help you plan ahead.
Rest & Compression
Swelling and bruising are normal. Wear your compression garment at all times. Light walking is encouraged but strenuous movement should be avoided.
Back to Light Work
Most men return to desk-based work within 1–2 weeks. Continue wearing the garment and avoid lifting, reaching overhead, or any chest strain.
Return to Exercise
A gradual return to physical activity can begin after 4 weeks. Chest exercises and heavy lifting should be cleared with Mr Juma before resuming.
Final results are typically visible within 3–6 months, once all residual swelling has fully resolved. Many men notice a meaningful improvement in the chest’s appearance well before this point, once the compression garment comes off at 6 weeks.
Candidacy
This procedure is suitable for most men who are in good general health and have persistent breast enlargement that has not responded to lifestyle changes. Mr Juma will confirm your suitability during a personal consultation.
